Before you can get in the water it's necessary to dive into forms! Forms are part of the dive lifestyle, but there's no better time to complete the forms than at your leisure. We've transitioned to online forms. You will need to complete the all-in-one waiver below. If you answer yes to any of the medical questions you also need to download the form associated with your class and have it signed by a medical professional, with no exceptions.
